Waist-to-Height Ratio vs BMI: What Each Number Shows

Compare waist-to-height ratio with BMI, see what each calculation uses, and learn why the two screening numbers are not direct body-fat measurements.

Waist-to-height ratio and body mass index (BMI) describe different features. BMI relates weight to height. Waist-to-height ratio relates waist size to height and adds information about central adiposity, which means fat carried around the middle. Neither number directly measures body fat or diagnoses a condition. For adults within its stated scope, NICE recommends using the waist ratio with BMI.

BMI uses weight in kilograms divided by height in metres squared. Adults use fixed BMI categories. Children and teens need age- and sex-specific reference charts, so the adult comparison on this page does not apply to them.

Waist-to-height ratio divides waist circumference by height in the same unit. The result has no unit. NICE recommends the ratio as a practical estimate of central adiposity alongside BMI for adults with BMI below 35 kg/m². This adult comparison is not for pregnancy or a condition that affects height. Those situations need an appropriate professional assessment. Do not extend the categories to every person or situation.

Suppose an adult is 170 cm tall, has an 85 cm waist, and weighs 75 kg. The waist-to-height ratio is 85 ÷ 170, or 0.50. The BMI is 75 ÷ 1.70², or about 26.0 kg/m². The ratio describes waist size relative to height. The BMI describes weight relative to height. The two results are not percentages and cannot be subtracted from each other.

  1. Use the same unit for waist and height before you divide.
  2. Use kilograms and metres for the metric BMI formula.
  3. Keep each result with its raw inputs, date, and method.
  4. Read the two screening results as separate pieces of information.

NICE describes a ratio from 0.5 to 0.59 as increased central adiposity and 0.6 or higher as high central adiposity within the adult scope above. These are screening categories. A result does not prove how much visceral fat is present and does not provide a diagnosis.

Understand why the results can disagree

A person can have the same BMI as another person and a different waist-to-height ratio. BMI cannot distinguish fat mass from lean body mass, such as muscle and bone. It also cannot show where fat is located. The waist ratio adds a circumference, but it still cannot identify the tissue under the tape or measure fat around internal organs.

Research comparisons usually test associations with later outcomes across groups. A 2013 meta-analysis included 24 cross-sectional and 10 prospective studies. It reported stronger waist-to-height associations for several cardiometabolic outcomes, with substantial differences between studies. A non-peer-reviewed 2025 medRxiv preprint followed 495,911 UK Biobank adults for a mean 13.7 years. Compared with BMI-defined categories, waist-based categories had meaningfully higher population-attributable fractions mainly for adults with BMI from 25.0 to 29.9. This statistic estimates the share of cases in a population associated with the higher measurement categories. Differences for BMI of 30 or more were under 1 percentage point and were not statistically significant for multimorbidity and mortality. This observational population evidence does not make a home measurement diagnostic.

Make a later comparison repeatable

BMI changes when weight changes. Waist-to-height ratio changes when the waist measurement changes, because adult height is usually stable. A changed ratio can also come from moving the tape, changing its tension, measuring over different clothing, or reading at a different point in the breath. Keep the procedure consistent before you interpret a trend.

  • For the NICE method, place the tape midway between the bottom of the ribs and the top of the hips.
  • Read the tape after a natural breath out.
  • Record the raw waist, height, weight, units, date, and measurement site.
  • Compare repeated results only when the method is the same.
  • Use a qualified health professional when a result will affect a personal medical decision.

Is a waist-to-height ratio of 0.50 the same as 50% body fat?

No. It means waist circumference is half of height. The ratio does not measure body-fat percentage or visceral-fat area.
